Does a Geng Hour Stem necessarily mean a Geng-Shen hour?

No. Geng’s Lu is Shen, but the actual Hour Branch (the lower symbol in the birth-hour column) comes from birth time; Geng-Zi or Geng-Yin can also be hour pillars. The phrase Geng Hour Stem does not automatically add Shen as the hour branch.
